Meet Our Team
Shelley Leach
FNP-C
Shelley is the owner of 121 Medical Place and a lifelong resident of Mendenhall, MS. She completed her degree in 2009 at University Medical Center in Jackson, MS, where she was recognized as a Who's Who Among Students in American Universities and Colleges. She was also a member of Sigma Theta Tau International Honor's Society of Nursing. Since graduating, Shelley has completed many hours of continuing education to stay up to date on current medical knowledge. She is a member of the American Academy of Nurse Practitioners, Mississippi Association of Nurse Practitioners, Mississippi Nurses Association, Sigma Theta Tau International, and American Academy of Anti-Aging Medicine.
Shelley is married to Willis Leach. They have two children, Will and Wade, that keep her busy. In her spare time, Shelley loves to travel, especially to Disney World.
Jessica Tolar
FNP-C
Jessica has been an FNP since 2019. She is a graduate of the University of Mississippi Medical Center. Prior to becoming a FNP, she was an RN for ten years. She is a member of the American Academy of Nurse Practitioners.
She resides in Brandon, MS with her husband Brenton. They have two children, Nora and Landon. Jessica's hobbies include camping at the lake with her family and all things MSU!
Heather Reid
LPN
Heather is a 2017 graduate of Copiah Lincoln Community College, where she received her LPN degree. She lives in Puckett, MS, with her three children, Jake, Addison, and Colton. When she isn't at the baseball/softball field or at a pageant with Addison, Heather enjoys going to the beach and spending time with her family.
Leslie Green
Receptionist
Leslie earned her certificate in medical billing in 2013 from Copiah Lincoln Community College. She is a lifelong resident of Mendenhall, MS where she lives with her husband, Hunter, and their two children, Reilyn and Brooks. Leslie enjoys going to concerts and spending time with her family when she isn't working.
Jasmine Ham
Medical Assistant
Jasmine is a 2016 graduate of Copiah Lincoln Community College. She is a lifelong resident of Mize, MS, where she lives with her two children, JD and Matti. She enjoys spending time with her family and friends when she isn't working.
